JOB PURPOSE
To assist the Accountable Care Strategy (ACS) in the following:
- Support the operational needs of NHG membership programme
- Develop and operationalize a functional and effective Population Health Data Mart (PHDM)
- Collaborate and coordinate with various stakeholders to acquire, consolidate and build critical and reliable population data in the PHDM for managing membership programme and population health
- Administer relevant training and familiarization for users of PHDM
- Work with corporate IT/data protection stakeholders to ensure legitimate and secure sharing and use of data
- Support the operational needs of membership programme
- Assist in initiatives related to Membership Programme e.g., extraction of membership data from CRM and send to printer for fulfilment of welcome packages
- Extract data and track fulfilment by eligible members to receive rewards, and track NHG Cares app utilization
- Update merchant offers and programmes on CRM
(B) Manage population HEALTH DATA MART (PHDM) development & operations
- Assist in prioritising resources, including budgeting work, for PHDM development and operations
- Conduct hands-on basic data analysis and reporting
- Manage and maintain ongoing PHDM operations, including (i) creating / updating data dictionary, (ii) assisting PHDM account application, (iii) assisting data re-identification process, (iv) assessing and coordinating data requests and uses for various population health programs and interventions by stakeholders, both internal and external.
- Participate in user acceptance test and any verification testing related to PHDM development and implementation
(C) COLLABORATE AND COORDINATE WITH STAKEHOLDERS ON POPULATION HEALTH MANAGEMENT
- Identify and monitor needs of various stakeholders for data
- Assist in developing and implementing sub-systems, policies, SOPs and workflows to govern and enable appropriate use of data, such as governance manual and related documentation
- Assist to implement audit protocols and to ensure compliance with data protection policies and rules.
(D) SECRETARIAT FOR TEAM AND DEPT MEETINGS
- Secretariat support including collating attendance tracking matters arising and recording discussions in a concise and timely manner.
- Collaborating with various stakeholders for inputs on discussion points
JOB REQUIREMENTS
Education/Training/Experience
- Any recognized basic degree. Qualifications in Health/Life Sciences, Public Health, Data Infrastructure, Health IT and/or Project Management would be an advantage.
- At least 2 years of relevant working experience in healthcare, project management or health IT development.
- High proficiency in Excel, Powerpoint and Word.
- High proficiency in managing large databases and registry operations.
- Proficient in engaging and managing multiple stakeholders.
- Proficient in Oracle Analytics Server (OAS) or any population management health tools would be an advantage.

- JOB PURPOSE
- The Executive Assistant will be part of the Community & Integrated Care (CIC) Operations team to provide operations and administrative support for the following CIC Programmes (including but not limited to) to support the population health work in the North Western Integrated Care Organisation (NW ICO):
- Hospital-to-Home (H2H)
- Medical Home (MH)
- Nursing Home Supportive Care (NHSC)
- The role will also include providing administrative support to the CIC Multi-Disciplinary Team (MDT) including but not limited to Doctors, Community Nurses, Allied Health Professionals, Pharmacists and Medical Social Workers, in planning, implementing and evaluating the CIC programmes
- MAJOR DUTIES & RESPONSIBILITIES`
- Specific
- Follow up closely with patient/patient’s next-of-kin who have been referred to CIC Programmes
- Schedule the first appointment for home visit and/or tele-consultation
- Updating and tracking of patients’ first appointments e.g. actualisation of first home visit/tele-consultation, rescheduling of first home visit and/or tele-consultation
- Manage phone and email enquiries
- Plan and plot CIC MDT’s rosters and calendars for beyond office hours’ duty, triage duty, MDT case discussions, scheduling of CIC patients’ appointments for home visit and/or tele-consultation, pandemic outbreak planning etc.
- Work closely with CIC MDT and community partners to support the actualisation of patient’s first appointment
- Preparation of patient case file for first home visit and/or tele-consultation,
- Appointment reminder calls to patients
- Manage referrals from clinical departments and/or other healthcare organisations
- Perform checks and update patient means-testing status, and submission of financial schemes/assistance applications for patient billing/subsidy claim processing
- Work with Patient Billing Services/Finance in checking and closing of cases for billing and processing of bill refunds (if any) so as to ensure patients are charged accordingly
- Procure, track and manage the department consumables inventory stock, medical equipment and resources required by CIC MDT for the CIC programmes
- Track CIC MDT’s claims for submission to MOH, NHG and Woodlands Health’s Senior Management (whichever appropriate)
- Manage the maintenance/fault-reporting of medical and non-medical equipment belonging to the department with the relevant departments/vendors. (e.g. collection of blood glucose monitoring devices for maintenance service rendered by vendor)
- Support room set up and preparation of materials/equipment for CIC programme related telehealth/teleconsultation sessions
- Work with HR and respective leads of the CIC MDT to provide administrative support for the on-boarding and off-boarding of CIC MDT members (e.g. applying of required system access.
- Support the collation of data/statistics for tracking, analysis and reporting purposes.
- General
- Provide administrative support to all CIC programme related meetings / training sessions, including but not limited coordinating, scheduling meetings/training sessions, sending invites for all meetings/training sessions, book room, prepare room and set up zoom etc.
