The Assistant Manager will support in the work of facilitating, enabling and driving care integration across NHG – covering Central & North zones, with focus on enabling the integration of care protocols and bundles that meets the objectives & outcomes required for our population.
The person will be working closely with an expanded cluster-wide team comprising of multi-disciplinary team members. Hence he or she is expected to be a team player, working closely with various zonal colleagues, as well as liaising and collaborating stakeholders like the Ministry of Health, Agency for Integrated Care, Health Promotion Board, related government agencies, voluntary welfare organisations, community development councils and grassroots leaders.
Major Duties and Responsibilities
- Supports the coordination and integration of new and existing providers (internal & external) onto agreed integrated and bundled care plans, with the intention to pool resources to deliver and improve health and outcomes.
- Supports the collaboration and work with Group Integrated Care departments in identifying and enabling key levers such as health and social integration to achieve optimal patient wellness and outcomes.
- Collates and reviews key updates of the GIC teams for biweekly updates update with DyGCEO.
- Administrative support to NHG Committees/Workgroups and GIC meetings, including in the provision of secretariat support which will includes curating agenda with the Chairperson and take minutes of the meetings.
- Any other special projects, as and when assigned.
Job Requirements
- A team player who is resourceful, hands-on, detail-oriented and able to manage multiple projects simultaneously.
- Must have positive attitude and excellent interpersonal, communication skills and organizational skills.
- Strong problem solving skills with the demonstrated ability to identify research and make decisions based on the day-to-day complex customer/prospect problems.
- Degree holder with an ability to write and communicate well. Minimum 5 years’ experience supporting public healthcare sector.
