Sr Staff Nurse II (Community Health)

The Challenges

You will promote health and prevent illness in the community by collaborating and coordinating with different professionals and community partners

1) smooth delivery of patient care and transition back to the community

2) involve in care planning, case management of residents in the Central Health zone.

You will also involve in early intervention for pre-frail senior identified in community, chronic disease management for patients whose condition are not well controlled and care of patient in their immediate post discharge and end of life care at home.  

The Requirements

• State registered nurse with at least 2 years of clinical experience
• Good interpersonal and communication skills, with ability to work as part of a multi-disciplinary team (including community partners)
