The Executive / Senior Executive will be responsible to assist in planning and implementing to new medical programmes and services for the patient care continum in YCH to the community as well as to support Yishun Health Unified Care Model (UCM) structure.
Major Duties and Responsibilities
- Assist in the development and implementation of the new clinical programmes, UCM care paths and the required data support system.
- Assist in connecting and collaboration with the relevant partners both internal and external organisations to implement the new inpatient service programmes.
- Collate and analyze key performance indicators and statistical reports.
- Provide support to respective Head/Deputy Head/manager of clinical departments on improvement initiatives and patient-centric work processess.
- Assist in the development and implementation of the care bundles
- Provide support to various projects and activities that streamline work processess and increas productivity.
- Assist in IT development and programme such as development of EMR and NGEMR
- Coordinate on-the-job programs for new staff: identify training needs, plan and organize programs and schedules, continuously evalutate and monitor staff progress, initiate reviews and feedback session when necessary.
- Responsible for monitoring and controlling identified risk
- Manage patient information with confidentiality and professionalism
General
- Support the alignment of the department’s goals with YH campus goals
- Support staff development and learning.
- Support corporate activities
- Perform additional duties as assigned by Management
