Major Duties and Responsibilities
(A) SPECIFIC(80%)
I. Develop a partnership with patients, assess and anticipate care requirements, make plans and ensure that patients receive appropriate care and services.
II. Achieve anticipated clinical outcomes and involve the care team by providing information and updates on patients’ treatment progress.
III. To familiarise with the community resources, initiate and coordinate referrals between patient and community partners.
IV. Perform data management, collating and maintaining of database for clinical and project submission.
V. Adhere to professional standards and integrity as outlined by protocols, rules and regulations.
VI. Document all case information, forms and reports accurately.
VII. Support the department in development and goal achievement.
Specific Areas of Responsibility
– Accomplishes patients’ care by assessing treatment needs, developing individualised care plan, monitoring and evaluating treatment plans and progress.
– Identify, analyse and find the causes and make adjustment to the patient’s care plans and goals which have not been achieved.
– Educate and provide adequate information to ensure patients understand their condision(s) and treatment needs.
– Facilitate/Coordinate interdisciplinary meeting and treatment plan for patients’ recovery.
– Coordinate the care delivered to the patients and facilitate communications between care providers and patients.
– Update multi-disciplinary team on patients’ progress and matters that affects patients’ recovery.
– Implement any required corrective measures to the clinical treatment plan with the multi-disciplinary team.
– Facilitate the plans for patients’ discharge with the multi-disciplinary team and coordinate the plan with patients.
– Promote and enhance the development goals, objectives and systems to support the department’s objective and goals.
(B) GENERAL (20%)
I. Promote the importance of Mental wellness in the hospital and community.
II. Develop a strong cohesive team within department and hospital.
III. Promote a patient- orientated culture that emphasizes in quality, continuous improvement and efficient services.
IV. Enhance overall hospital services and development.
V. Perform tasks as directed by Manager, Program Director and Head of Department Psychological Medicine.
