JOB SUMMARY
- To assist in ensuring smooth operational management and continuous improvement of the assigned Track(s), operational unit(s)/area(s), and/or projects, in the Network Development arm of the Division of Central Health (DCH), which consists of 3 main tracks: Partnerships, Digitalisation and Finance
- To assist in ensuring good collaboration and partnership with internal and external stakeholders in providing holistic care for the community and patients.
- To support the Director/ Deputy Director/ Assistant Director, DCH’s Network Development in ensuring that all relevant Key Performance Indicators (KPIs) are met for the above.

MAIN DUTIES AND RESPONSIBILITIES
Strategic Partnerships
- Event lead for Central Health Action & Learning Kampung (CHALK) 2023. To provide support to DCH leaderships in conceptualizing and planning for the event. To ensure smooth execution of the event so to deliver a positive learning experience for Central Health’s target audiences.
- Provide DCH support to internal stakeholder so as to ensure adherence by project owners to agreements management framework and orderly maintenance of partner database and Partner Relationship Management System (formal and informal collabs).
- Support in interface with internal and external groups and/or stakeholders so as to strengthen care integration and Communities of Care (COCs).
- Support DCH leadership in strategic leadership engagement, by steering project/committee discussions with partners towards agreed Central Health strategies set out at Senior Management level
Primary Care Partnerships
- Support Central Health as a GP Relationship Manager to engage GPs in Central Health to onboard and support Healthier SG and its enrolment scheme.
- Provide oversight in HSG Letter of Agreements & GP partnership programme agreements. To ensure orderly maintenance of GP partner database and Partner Relationship Management System.
- Drive sub zonal level engagement activities, to create platforms whereby multi-organisation partners and Primary Care partners could interact and provide care link ups within a COC.
- Support in the implementation of GP Partnership Programmes (Community Right Siting Programme/ Shared Care, GPFirst/GPNext)
- Lead/ support GPBUZZ editorial team to produce publications that educates GPs on community health, latest development in TTSH & Central Health and introduce collaboration opportunities.
