Job Purpose
Group Integrated Care (GIC) drives and supports the strategic development and implementation of NHG’s Population Health Strategy, in synchronisation with the MOH Healthier SG (HSG) strategy. GIC achieves this through driving and coordinating integrated care planning and programmes, new clinical capabilities, finance and performance transformation, partnerships and engagement, and key enablers such as digital enablement and workforce transformation. It also supports the development and operations of NHG Cares and NHG’s Integrated Care Organisations (ICOs) serving their respective population zones (Central, Yishun and Woodlands).
As a key member of the GIC-ICO Planning team, the incumbent shall work to ensure smooth and effective engagement, implementation and evaluation, in accordance with NHG’s objectives and developed strategies under Integrated Care Organisation (ICO) track. The focus of the ICO work is to develop and ensure smooth operations of the ICOs serving their respective zones and through the ICOs, drive care and finance transformation and digitalisation for integrated care and relationship based care. This will also drive the performance of NHG Population Zones in achieving NHG’s Population Health Aims.
The scope shall include but not limited to driving the development of department/division strategies, developing plans and facilitating discussion to empower ICOs to implement the plans in line with care and finance transformation, resource acquisitions, internal/external stakeholder management as well as overseeing the execution with accountability for the approved milestones/budgets/timelines and in compliance with directions given by the Group Director.
AREAS OF RESPONSIBILITY
1. Population Health Strategic Planning and Management Platforms. Work collaboratively with the GIC Operations Planning Team, and lead in assigned areas, to drive, integrate and support the design and implementation of the strategies, operations planning and change management for the NHG Population Health Transformation, in close association with internal and external stakeholders
2. Healthier SG (HSG) Implementation. To develop and plan NHG implementation of HSG in coordination with the ICOs, particularly for HSG operations, resident engagement for enrolment and enablers at the cluster level, in alignment with MOH and key partners; In the process, function as a key point of contact between MOH and NHG on matters relating to HSG, and across the ICOs and relevant NHG units.
3. Digital and Data Transformation. To assist in conceptualising/ updating a viable digital and data business strategy that supports NHG Population Health efforts. The work involves efforts to (a) gather, align and integrate the business requirements and use cases while balancing the interests of diverse users and stakeholders for both digital solutions and data needs; (b) To scope and develop digital products which are essential for business requirements and achievable by the NHG Population Health milestones timeline; (c) To review and secure access to data required for population health.
Job Requirements
- A team player who is resourceful, hands-on, detail-oriented and able to manage multiple projects simultaneously.
- Must have positive attitude and excellent interpersonal, communication skills and organizational skills.
- Strong problem solving skills with the demonstrated ability to identify research and make decisions based on the day-to-day complex customer/prospect problems.
- Degree holder with an ability to write and communicate well. Minimum 5 years’ experience supporting public healthcare sector.
