Admissions Coordinator – LTC

Location: Good Samaritan Center

Job Description:

• Ensure LTC patient admission applications and insurance information are complete
• Obtain relevant medical records
• Confirm insurance information / payment sources
• Work closely with interdisciplinary team to ensure smooth patient transition at admission
• Excellent communication and customer service skills required**
• Accurate records of admissions and discharges required**
• Knowledge of LTC insurance, Medicare, Medicaid, and private pay options required**
