Family Group:
As the Senior Executive / Executive (Service Planning) of Corporate Development, you will assist the Senior Manager / Manager of Corporate Development, Yishun Health, in the development and deployment of policies, systems and processes, and lead the adoption of evidence-based practices to enable a holistic approach to value-based care across the whole of Yishun Zone. You will also assist in planning new clinical services within Yishun Health in alignment with the Yishun Zone Unified Care Model (UCM).
Detailed Scope
1. Define and enable value-based care using the Yishun Zone model of care through the following:
a. Development of value-based performance framework and indicators.
b. Development of tools/ criteria for defining the Yishun Zone population, including but not limited to population needs assessment tools and segmentation tools.
c. Development and driving the adoption of performance and cost measurement tools using value-driven care methods.
2. Support continuous improvement of value through:
a. Supporting the development of logic models in Yishun Zone at macro, meso and microsystems levels.
b. Working with operations and finance to standardize care/ work processes and costs.
c. Building enablers and increasing system-level capabilities and capacity to scale up value-based care approaches, including working with HRD to develop and implement appropriate curricula to accelerate the adoption of value-driven care methods.
3. Assist in the necessary systems and services evaluation, research, surveys or studies where required.
4. Perform Clinical Services Planning & Development by:
a. Assisting senior clinicians in planning new clinical services in alignment with YH priorities and UCM.
b. Working with various stakeholders (including but not limited to operations, finance, senior management, etc.) to develop a coherent clinical service plan.
5. In addition to the above, you will assist D/CD, DD/CD and AD/CD to ensure Corporate Development sustainability to meet the needs of Yishun Zone and its institutions through the following:
a. Conduct learning development and performance management for relevant junior staff of Corporate Development.
b. Coaching and continuous development of any other junior staff in Corporate Development.
c. Embrace and foster continuous improvement as our way of life by improving and refining work performance and encouraging staff efforts to improve processes in achieving clinical quality and operational excellence on a daily basis.
6. Responsible for assessing and identifying any potential risk that may hinder the reputation, safety and financial well-being of Yishun Health.
7. Liaise with external agencies, for example, Ministry of Health and NHG institutions in work streams related to Value-Based Care.
8. Carry out any other work deemed relevant by the Director, Deputy Director/ Assistant Director, CD and accountable for the results and deliverables.
